It has a section for your customer's name, address, insurance information, automobile information, etc. It allows you to easily total up the charges to bill your customer. In addition to a payment information area it includes a statement of authorization and satisfaction and a customer signature line.

Available on 2 Part White-Yellow or 3 Part White-Yellow-Pink carbonless(NCR) paper. This paper copies whatever is written on the top sheet to the other sheet(s), allowing you to have multiple copies of your filled out form.
